Slider for surcharge creation on item cards

Can a slider be added to item cards that when toggled creates a mirror of the part number with a prefix of SUR and a automatically assigned to the Surcharge category.
Ideally these should be linked so whenever the parent number is added to the screen the surcharge is also automatically added but left so the surcharge can be added alone for credit purposes.
